How do I cancel or reschedule an appointment? Follow the instructions below based on where your appointment is scheduled. Another SNHD employee, or employee of the business being served, must be present as a witness at the time the phone call is placed. Confirmation of the identification of the person on the phone is done and permission is documented in the nursing notes, and ed by the nurse and the witness.
